The add-on introduces a dedicated Quiz Field found under the Advanced Fields section of the Gravity Forms editor.
Supported Input Types:
Radio Buttons: Single-choice answers.
Drop Down: Single-choice answers in a collapsed list.
Checkboxes: Multiple-choice answers (allows for “select all that apply”).
Choice Management:
Randomize Order: Automatically shuffles answer choices for each user to prevent cheating.
Correct Answer Marking: Simply click a checkmark next to the correct choice in the editor.
Answer Explanations: Provide “Instant Feedback” or post-submission explanations for why an answer was correct or incorrect.
The plugin offers three distinct ways to evaluate a user’s performance:

| Pass/Fail | Set a minimum percentage (e.g., 80%) required to pass. Displays a custom “Pass” or “Fail” message. |
| Letter Grade | Define custom ranges (e.g., 90-100% = A, 80-89% = B). Fully customizable labels and thresholds. |
| No Grading | Useful for informal quizzes where you only want to collect data without a final score. |
Weighted Scoring: Instead of 1 point per correct answer, you can assign custom values (e.g., 5 points for a “perfect” answer, 2 points for a “partially correct” one).
Negative Scoring: You can assign negative values to incorrect answers to penalize guessing.
Once entries start coming in, the plugin provides a dedicated Quiz Dashboard within the WordPress admin.
Aggregate Data: View the average score, pass/fail rate, and grade frequency across all participants.
Question Breakdown: See which questions were most frequently missed to identify gaps in your content or training.
Individual Entries: View a specific user’s submission with their total score, percentage, and grade clearly displayed.
License Level: Requires an Elite or Nonprofit License. (It is not included in Basic or Pro plans).
Prerequisites: Must have the core Gravity Forms plugin installed and activated.
